Caregiver Homes is a provider of the Massachusetts Adult Foster Care Program. Caregiver Homes supports Consumers 16 and older who are frail or have a chronic disability, are Mass Health eligible, and who are receiving daily support with Activities of Daily Living from a live-in primary caregiver.
Caregivers are often family members, and cannot be a spouse or legal guardian. Caregiver Homes provides Assessment, Coaching, Education, Collaboration, Case Management and Referrals to Caregivers and Consumers through monthly home visits and daily online communication. Caregiver Homes employs Registered Nurses and Care Managers to work with families in their home setting.
All caregivers receive a tax-free daily stipend for the care they provide to their consumer daily. Caregiver Homes is proud to have a dedicated RN, CM team who live on Martha’s Vineyard and bring years of experience to their consumers and caregivers in the community.
