Martha’s Vineyard Community Services Counseling Outreach and Referral for the Elderly (CORE) program works with at -risk and mostly home-bound elders in the community. The program responds to referrals from the Island’s Councils on Aging, First Stop, VNA, physicians and other professionals as well as self-referrals.
CORE’s goal is to meet the needs of those who might otherwise avoid treatment due to fear and inability to travel to the Island Counseling Center for treatment. CORE provides coordinated care through a combination of community, office and home based behavioral health and supports to people 55 years and older. CORE actively engages elders in individualized services that: enrich their lives; keep them healthy and safe; and involve them in the programs and activities of the community.
What services does CORE offer?
For people 55 and over CORE provides free mental health assessments, short term counseling, care coordination, home visits, referrals to available resources, support groups and referrals for psychiatric visits at Island Counseling Center (ICC). People who are unable or reluctant to come to the Council on Aging (COA) or the office may receive counseling at home.
What is Care Coordination?
Care coordinators work with people to help manage important appointments and tasks. When organizing and connecting is difficult, care coordination promotes safe and healthy lifestyles by assisting with access to available resources including:
- Transportation
- Medical Appointments
- Councils on Aging
- Community programs and groups
No one should avoid medical care out of fear or inability to coordinate logistics of going off-Island.
CORE Care Coordinators connect clients to services that support their quality of life, ranging from transportation needs to food resources and beyond.
No one should have to choose between paying a counseling sessions co-pay or using that money for food or other necessities.
CORE addresses significant inter-related physical and behavioral health, social and case management needs of elders at no cost to the client.
