Horizons Geriatric services include:
- Comprehensive functional and mental health assessments
- Home safety evaluation
- Coordination of Medical appointments
- Medical oversight to include proper use of medications/treatments
- Mental health counseling for the elder and his/her family
- Facilitation of legal assistance for Health Care Proxies, Estate Planning, Guardianship and other legal matters
- Coordination and supervision of outside services
- Education regarding alternative residential living options as needed
- Facilitation of information between elders, family and medical teams
